05.12.2010 - CT Scan with Dye - EKG - Blood Culture - Urine Testing

The following highlights what happened before I was diagnosed with Blood Clots in my ankles and upper chest region resulting in shortness of breath and also the pain caused by a kidney stone in my right side and lower back!

1.  About four weeks ago I had a pain in my right side and lower back but that passed after about three days.   I thought that was due to straining a muscle at the YMCA.

2.  I felt out of it for a few weeks and thought it was a touch of flu as I had aches in my ankles and felt tired.

3.  Two weeks ago I was walking to the YMCA which is four blocks away.  I walked a block and suddenly had shortness of breath and had to sit down for five minutes before walking another block.   This went on for a few days but then felt somewhat better.

4.  I didn't go the the Emergency Room as I had an appointment with my doctor (Dr. Vallabhaneni) at the VA Hospital on Tuesday, 05.11.2010.  I went in at one p.m. and she had me take a urine test and a blood culture and I went home.   She called me around four p.m. and said the results had come back already and I was to come in on Wednesday for more tests and my kidneys and blood culture looked strange.

5.  On 05.12.2010 I went in at nine a.m. and Cindy, the lead nurse in the infection disease center said I needed another urine test and blood draw.  I then went to the Scanning Unit and they put an IV in my left arm and put in a drip of Saline Solution for my kidneys and after two hours, put me in the CT Scan and fed me the dye in the IV. 

After the scan they kept me in the scanner and gave me the rest of the Saline Solution.   Then they sent me back to the Scanning Center and back to a blood draw and urine sample.  I went back they said I was through, I said are you sure, I thought I was to talk to the nurse, Cindy.  No they said and took out the IV and sent me home.  I got home at five p.m. and after about five minutes the phone rand and Cindy asked why I was home. 

They had a hospital bed ready for me and were waiting for me so I caught a bus and got there by seven p.m. and they hooked my IV up to Saline Solution and a Heparin - (Blood Thinner) due to the multiple clots in my chest plus they gave me medication for my prostrate to hopefully help me pass the kidney stone.  Plus they put me on oxygen!

6.  So starting on 05.13.2010 they drew blood four times a day, did at least one EKG a day, IV drip of Heparin and four times a day took blood pressure, oxygen levels and temperature.

7.  On 05.14.2010 they sent me to Ultra Sound and did a measurement on my legs and determined I had blood clots all over my upper chest region.  I had an ultra sound on my legs three years ago and they were fine.  The doctor's thought perhaps the medication I was taking for my Ocular Myasthenia (Double Vision) was causing the blood clots so they took me of the medication and substituted a new medication.

8.  At stage on 05.15.2010 they said not to worry as I had the blood thinner working and I probably didn't have to worry about the clots breaking off and hopefully in six month or less, they would be reabsorbed into the body.  They said we might never know what caused the blood clots but as long as we took care of them, not to worry.

They said I must have a strong constitution as most people would have gone under six months to year before I did.   They think the exercises I do every day at the YMCA, five miles on the bike, half an hour of yoga and weight lifting, helped me a great deal.

9.  Some bruising from the Heparin and the continuing testing of urine, blood and EKGs.

10. On 05.15.2010 they only drew blood twice a day and moved my to the second floor as I was getting better.

11. Today was the same as the last few days but my urine looked very dark.  Otherwise I was fine.  Then my urine cleared up!

12. On 05.19.2010 my lower back started to hurt me and the doctor asked if I wanted some pain medication.   I usually don't take it but thought, why not.  Well by two p.m. I felt fine.   I told the doctor I loved the meds and then around four p.m. I went to the bathroom and the urine was dark and there were spots floating around so I captured them and put them into a sample container.   The next hour the urine was clear again.  Strange!   Then around six p.m. I went again and there was blood in the urine and on the urine container.  I looked into the container and there was a kidney stone that was over a Quarter of an Inch across!  I showed the nurse and she couldn't believe it, it is BIG she said.  It got around and people from nurses to the blood draw personnel came in to look at it and - I didn't have any pain passing the stone.  Go figure.  Take pain pill I think!

13. Today they took off the Heparin drip and also the monitoring cables from my chest that was connected to the nurses station to ensure my heart was functioning correctly.    I was happy but... they said I would have to give myself injections twice a day of Heparin until my blood level was at a lever 2.02.   I said what!   Well that night the nurse did it for me.

14. Well it is 05.21.2010 and I will be leaving this afternoon but the nurse said in the morning I had to give myself the Heparin injection or else.  

Well I did it, pinched the fat on my stomach and rubbed with alcohol and jammed in the needle.  No pain! 

My friend Patti said she would pick me up and around 2:30 p.m. they took off the IV and Patti picked me up and was nice enough to drive me to the Safeway where I bought enough food for a few weeks.  She said if I needed moral support to call her, she would love to give me a hypodermic needle in the stomach!

So this evening I did the pinch on the other side of my stomach and did the deed!   No pain, no gain.

15. This weekend I will be giving myself the Heparin shots in the stomach twice a day and at night taking two Coumadin Pills for the blood thinning.

16. Monday, 05.24.2010 - I will go to the VA and get a blood draw and be diagnosed at the Anti-coagulation Unit to review the blood draw.

Well I got to the Blood Lab at eight a.m. and proceeded to get the blood draw.  Ben, a lab technician was there and had a good laugh as he took many blood draws during my stay in the hospital and also enjoyed seeing my huge kidney stone.     I went to the Medical Practice Unit to meet with the nurse practitioner Linda Fong.  Well she said I shouldn't get too excited as there was good news and bad news.   Excuse me.   She said I had a blood clot in my left leg, right leg and upper left chest and multiple ones in the right chest area. 

Then she said my blood count as far as clotting was concerned on the Coumadin and the Heparin shots in the stomach, did match to the needed quota of between 1 and 3.   My level today was a 3.    She said I should cut down the Coumadin but to cut down the second pill in half (7.5 milligrams instead of 10 milligrams) and do the Heparin shot in my stomach to only tonight, not tomorrow morning.   Linda said she wanted me to follow a strict diet as to what I have been eating and I told her my diet and she said that was fine.   Not too many drinks she said. 

She wants me to come in again tomorrow for a blood draw and to meet her and then decide if I need to continue the Heparin shots in my stomach.   In the afternoon I meet with my General Practitioner (Dr. Vallabhaneni) to see how I am doing.

I also asked about my vacation and Linda said as far as my blood level, if in the 2 to 3 level, no problem.   She does want to see me twice the week before I leave to Yellowstone.   She also checked out my support hose and said they were not strong enough and ordered new ones and will have them mailed to me.  She said wearing them on a flight would be the best thing and also maybe in the future on long flights across either ocean, to wear the support hose to help eliminate problems with blood pooling after sitting too long.

17. On Tuesday, 05.25.2010 - I went to the VA first have a blood draw and then to see RN Linda Fong.  I got the blood draw at 10 a.m. and then proceeded to see Linda Fong at 11 a.m.  When I got there a technician called my name and took me to a different office - I thought that was strange!   She said they were doing a second blood test, I looked concerned!   She laughed, said they have a computerized system, she took a drop of blood from my finger, put it on a plastic tray and slid into a box about ten inches long by four by four inches with a touch pad.   A few minutes went by and low and behold, the blood clotting measurement showed up.  She keyed in my social security number and the results went to my data base!   She said the blood draw showed me at 2.9 - yesterday it was 3.0.   Rather high.  I then met with Linda and she said I am off the self injections in the stomach but I should hold onto the three hypodermic needles with the Heparin as they cost $50.00 each!  I am just on Coumadin for the time being as long as I stay in the safe zone for blood thinning!   She said I along with one other patient was doing just great.   I will go back on Friday morning to have the blood test in the unit plus meeting with Linda as to the status of my blood tests!  So far, so good!

 This afternoon l went to see my General Practitioner - Dr. Vallabhaneni.   She said she was amazed at my status and I should keep up the good work!   She is only there on Tuesday but she visited me twice in the hospital and that is unusual.   She really has concern for her patients!    And that was my Tuesday, next stop is Friday!   They said I could do some exercises but not to over do anything.   I should be fine by the time I go on the trip with Gail on June 14th!

18. On Friday, 05.28.2010 - I went in at ten a.m. and the nurse's aide took my blood draw and put the drop of blood in the machine.  It displayed my blood coagulation count as 3.6!  I was surprised as they had told me the level should be between 2 and 3.   Well I went in to see Linda Fong and she laughed, said I was doing great.   She said the first few weeks they wanted my blood lever to be around 3.5 and it was 3.6 so it was almost exactly what she wanted!    I told her my sister was paranoid and said I might have a heart attack if I took the trip to Yellowstone.  Linda had a good laugh and told me to tell my favorite sister - "We all are paranoid!" - and if my blood level was what she wanted on my next visit on Tuesday, June 1st, there wasn't any reason not to go on the trip.  She said to keep on the same regime of Coumadin and I should be fine.

19. On Tuesday, 06.01.2010 - I will go to the VA to see RN Linda Fong again to see what my blood level is.    Well it was 3.4, just where Linda wanted it.

She also sent me to get my pressure stockings as they were not mailed to me.  They said they were four weeks behind in mailings and I went directly to the clinic where they measured my legs and gave me four pair of light and two pair of heavy pressure stockings. 

Linda was also nice and wrote out a letter for my friend Gail, whom I will be traveling with to Yellowstone and the Tetons in two weeks.

20. On Tuesday, 06.04.2010 - I will go to the VA to see my neurologist, James Maas, MD.  He said I was doing great on the new eye medication and it looked like I had a mild case of Ocular Myasthenia Gravis as many after two years lose muscular functions slowly,  So far my Myasthenia hasn't progressed to a higher level.  So far my strength has been the same!  He also gave me a letter to  travel.

21. On Tuesday, 06.07.2010 - I will go to the VA to see RN Linda Fong again to see what my blood level is. 

My blood level was 3.4 so still high but getting where she wants it.   She wants it in the 2.0 to the 3.0 range before I go on my trip.

22. On Thursday, 06.10.2010 - I will go to the VA to see the Neurology-Muscular doctor about the meds and my eyes.

I met with the neurologist and she said I was doing fine.   She said my muscle strength was fine and she had talked to the top specialist in Ocular Myasthenia.  He said usually if one doesn't lose strength in their body muscles and if they can swallow without problems within two years of detecting Ocular Myasthenia, they probably will not have the type that will further deteriorate the body functions.    He also told her that in some cases after a few years, it is possible that the Myasthenia suddenly burns itself out and the persons eye site is back to normal.   So she said lets hope for the best and it seems I am doing great and should just keep up taking the current drug and doing exercises and so on.

23. On Thursday, 06.10.2010 - I will go to the VA to see RN Linda Fong again to see what my blood level is. 

I had the blood draw and the results were a 2.2 for the blood coagulation and NP Linda Fong said I was exactly on target and there shouldn't be any problems taking the trip to Yellowstone.  She said a few things to do, no liquor on the plane flights, wear the pressure stockings and walk a lot on the trip.   Champagne was just fine at night!  And just keep taking my Coumadin pills!

24. On Monday, 06.21.2010 - I will go to the VA to see RN Linda Fong again to see what my blood level is after my trip to Yellowstone and the Tetons.    I was balled out due to the fact that Gail and I had drunk multiple bottles of champagne!  Linda said she thought I was one patient she could actually trust and I was in BIG trouble!   But she said I was still doing great and my trip was a success without any problems.

25. On Monday, 06.29.2010 - RN Linda Fong said my coagulation of blood was at 3.1 and I told her I didn't have a drink all week.  She laughed and said one drink a day is fine! Told me to come back on 07.15.2010 for the usual blood test.

I also met with my primary doctor - Dr. Vallabhaneni this afternoon and she said I was amazing and was sorry as she was going back to the San Francisco General Hospital and this was the last time she would meet with me.  She is so nice and even came to the patient ward to see me on Thursday and Friday, her days off from the V.A. - so nice of her.

27. On Monday, 03.21.2011 - Another meeting with RN Linda Fong to check my blood level.  As you can see by the above listing, I have been seeing Linda on the average of about every four to five weeks.  I did take several vacations last year and this year I went to Panama for my 71st birthday.  So I am in good shape to travel and do what I want. 

Today my blood level was 2.4 - Linda said I was the perfect patient and to keep up the good work!
